Informed Consent Policy.

Counselling Services

Welcome to Mindful Counselling Australia and thank you for choosing us for your counselling needs. As part of providing a counselling service to you, your counsellor, will need to collect and record personal information from you that is relevant to your current situation.  This information will be a necessary part of the assessment and treatment that is conducted. This document (the agreement) includes important information about our services, policies and expectations. Please read it carefully.

 Access

You may access the material recorded in your file upon request, subject to the exception in National Privacy Act (1998).

Confidentiality

All personal information gathered by the counsellor during the provision of the counselling service will remain confidential and secure except when:

1.  It is subpoenaed by a court, or

2.  Failure to disclose the information would place you and/or another person at risk; or

3.  Your prior approval has been obtained to

a. provide a written report to another professional or agency. E.g. a GP or a lawyer; or

b. discuss the material with another person. E.g. a parent or employer.

Fees

Fees will vary depending on what services you choose to utilize. We will discuss fees prior to you attending the counselling or MBSR program. You may be eligible for private health rebates. You would need to contact your private health provider to find out if you are covered for counselling services and the amount that will be reimbursed to you.

Cancellation Policy

If, for some reason you need to cancel or postpone the appointment, please give me at least 24 hours’ notice, otherwise you will be charged a $30 administration fee.
